What You ll Do
  • Plan and execute rehabilitation programs that improve patients physical function.
  • Perform initial and ongoing physical therapy evaluations and set short- and long-term goals.
  • Deliver treatment using therapeutic exercise, neuromuscular re-education, physical modalities, gait training, and manual techniques.
  • Implement and adjust Plans of Care based on patient progress and evolving needs.
  • Document patient progress and maintain accurate patient records according to facility and medical documentation procedures.
  • Collaborate effectively with therapy staff, department staff, and referral sources involved in patient care.
  • Provide patients with discharge instructions and an appropriate home exercise program after Plan of Care completion.
  • Supervise physical therapy assistants, as indicated, in a consistent and professional manner.
What You Need
Required
  • Active Physical Therapist license (state licensure requirements apply)
  • Experience developing Plans of Care and delivering outpatient physical therapy interventions (details not specified)

Preferred
  • Strong experience administering a wide range of physical therapy techniques
  • Familiarity with documentation and care coordination expectations within outpatient settings
  • Experience collaborating with patients, caregivers, and interdisciplinary staff
