What You ll Do
- Plan and execute rehabilitation programs that improve patients physical function.
- Perform initial and ongoing physical therapy evaluations and set short- and long-term goals.
- Deliver treatment using therapeutic exercise, neuromuscular re-education, physical modalities, gait training, and manual techniques.
- Implement and adjust Plans of Care based on patient progress and evolving needs.
- Document patient progress and maintain accurate patient records according to facility and medical documentation procedures.
- Collaborate effectively with therapy staff, department staff, and referral sources involved in patient care.
- Provide patients with discharge instructions and an appropriate home exercise program after Plan of Care completion.
- Supervise physical therapy assistants, as indicated, in a consistent and professional manner.
What You Need
Required - Active Physical Therapist license (state licensure requirements apply)
- Experience developing Plans of Care and delivering outpatient physical therapy interventions (details not specified)
Preferred - Strong experience administering a wide range of physical therapy techniques
- Familiarity with documentation and care coordination expectations within outpatient settings
- Experience collaborating with patients, caregivers, and interdisciplinary staff
