Medical Director - Geriatric Medicine West Springfield, MassachusettsFull-Time Leadership Role Not Accepting Locums or Visa CandidatesPosition OverviewSeeking a dedicated full-time Medical Director to lead an innovative Program of All-Inclusive Care for the Elderly in West Springfield, MA.This position provides comprehensive medical oversight and care coordination for seniors with chronic medical needs, helping them maintain independence and live safely in their communities.Clinical Responsibilities
  • Conduct histories and physicals on new participants, as well as event-based and periodic reassessments
  • Develop individualized medical treatment plans and provide medically necessary services in various care settings
  • Interview and evaluate applicants and their caregivers; participate in Intake and Assessment decisions
  • Communicate medical conditions and prognoses to participants and caregivers, supporting informed care planning
  • Collaborate in nursing treatment programs and provide medical direction for care teams
  • Participate as an active member of the interdisciplinary leadership team to achieve organizational goals
  • Educate and advise team members on diagnoses, functional status, and treatment options
  • Lead and participate in team and family conferences and decision-making processes
  • Educate participants and families regarding chronic disease management and health maintenance
  • Supervise care and treatment plans developed by the interdisciplinary team
  • Order, interpret, and document laboratory and diagnostic tests systematically
  • Participate in provider on-call rotation as required
Qualifications
  • Experience: Prior medical director or geriatrics experience required
  • Board Certification: Required (Geriatrics or Internal Medicine preferred)
  • Licensure: Eligible for Massachusetts medical license
  • Locums Candidates: Not accepted
  • Visa Candidates: Not accepted
