Work Dates Needed: 4/1-5/29/2026
Preferred Schedule: Monday 7am-4pm , Tuesday: 8am-5pm , Wednesday: 7am-4pm, Thursday: 8am-5pm, Friday: 7am-4pm
Worksite Setting: Clinic
Scope of Work: Family Practice/Employee Health. This is a 1:1 center so provider will need to be proficient in Phlebotomy
Licenses, Certifications, Requirements: BLS, DEA, Active WI License. 3 years of experience as an advanced practitioner.
EMR: Epic
