What You'll Do: - Evaluate, screen, and rehabilitate patients with physical functioning disorders or wound care needs, developing personalized care plans aligned with all regulatory and clinical standards.
- Utilize standardized assessments and evidence-based practices to guide treatments, including manual techniques, therapeutic exercises, modalities, and assistive device support.
- Monitor patient progress, adjust treatment plans as necessary, and evaluate outcomes to ensure optimal recovery.
- Provide detailed consultations and counseling to patients, families, and caregivers, fostering a collaborative approach to care.
- Maintain thorough, accurate documentation of all interventions, assessments, and progress in our electronic systems in compliance with regulatory requirements.
- Collaborate effectively with the healthcare team in meetings, care conferences, and training sessions to ensure continuity and quality of care.
- Support and supervise rehabilitation aides, assistants, and students while adhering to professional standards set by the APTA and state regulations.
Required Skills: - Valid and current Physical Therapist license for Massachusetts
- Proven experience in outpatient, long-term care, or rehabilitation settings
- Strong assessment and clinical decision-making skills
- Excellent communication and teamwork abilities
- Ability to document accurately and efficiently using electronic health records
Nice to Have Skills: - Experience with wound care management
- Additional certifications in specialized modalities or recent continuing education courses in geriatrics or rehabilitation
- Ability to mentor or supervise support staff and students
Preferred Education and Experience: - Hold a Bachelor's or Doctorate in Physical Therapy from an accredited program
- At least 2 years of relevant clinical experience in long-term or rehabilitation care settings
Other Requirements: - Ability to work daytime hours on a flexible, part-time basis
- Willingness to travel within the facility to support patient needs if required
- Valid CPR/BLS certification (or willing to obtain)
