SNF Physician
Location: Long Island, NY
Overview:
Our established geriatrics group in Long Island, NY is seeking a SNF Physician to join our collaborative clinical team. Our role focuses on skilled nursing facility oversight, chart review, and coordination with midlevel providers and facility leadership. Our position offers a structured weekday schedule within a team-based environment focused on quality geriatric care.
Position Offers:
• Monday Friday schedule
• Competitive compensation of $275,000 $325,000 with performance and productivity bonuses

Key Responsibilities:
• Review and manage patient charts for quality and compliance in SNF settings
• Provide clinical supervision and support to midlevel providers
• Communicate regularly with skilled nursing facility leadership regarding patient care coordination
• Oversee clinical decision-making for geriatric patients in assigned facilities
• Support continuity and coordination of care across SNF settings
• Participate in team-based care planning and clinical oversight
Qualifications:
• Minimum 1 year of experience in skilled nursing facility (SNF) settings
• Active physician licensure in good standing
• Experience in geriatric care preferred
• Strong communication skills with clinical teams and facility leadership
• Ability to provide clinical supervision and oversight
