  • Continuity: The facility is looking for ongoing support; a provider with consistent monthly availability will be prioritized.
Provider Qualifications
  • Licensure: Active Arizona (AZ) Medical License.
  • Board Status: Board Certified (BC) in Interventional Cardiology.
Credentialing Speed: 14-day turnaround.
