Schedule: Monday Friday Full-Time
Practice Setting:
- Comprehensive outpatient PACE program for seniors with chronic health needs
- Interdisciplinary team environment
- Community-based care to maintain patient independence
- Services include primary care, therapies, nursing, home support, and more
Role Overview:
- Medical oversight of participant care
- Clinical supervision of PCPs and APPs
- Leadership of QAPI and quality measures
- Participation in intake assessments and care planning
- Ongoing evaluation and management of chronic conditions
- Guidance to interdisciplinary care team
- Medical policy development
- Limited on-call (PACE model; discussed at interview stage)
Documentation System/EMR:
Patient Population:
- Adults ages 55+ with nursing-home level needs
- Community-based elderly population
- Mix of medically complex and chronically ill seniors
- No inpatient rounding required
Requirements:
- Active Massachusetts Medical License Required
Board Certified in Internal Medicine or Family Medicine Required - DEA Required
- Minimum 1 year of experience working with frail/elderly populations Required
- Prior supervisory/leadership experience Required
- Geriatrics experience or added qualifications Preferred
- Excellent communication and interdisciplinary leadership skills
