This role offers the opportunity to serve diverse and underserved populations while working collaboratively with physicians and multidisciplinary care teams.Key ResponsibilitiesPerform comprehensive patient evaluations, including history and physical examinationsDiagnose and manage acute and chronic medical conditionsOrder and interpret laboratory tests, imaging, and other diagnosticsPrescribe medications and therapeutic interventions within scope of practiceDevelop and implement treatment plans in collaboration with the Medical DirectorMonitor patient progress and modify care plans as appropriateInitiate emergency procedures when clinically indicatedProvide preventive care, health education, and counseling servicesMaintain accurate and timely documentation in accordance with agency and regulatory standardsCollaborate with physicians and other healthcare professionals to ensure coordinated, high-quality care
