You will focus on identifying clinical needs and chronic conditions to ensure patients receive proper follow-up from their PCPs.Daily Volume: Conduct 56 comprehensive assessments per day.Focus Areas: Perform annual risk assessments, chronic condition reviews, and social determinant screenings.Diagnostics: Perform A1C testing when applicable; no other invasive screenings are required.Care Coordination: Identify and escalate urgent findings directly to the patients Primary Care Provider.Prescribing: None.
