Active UT License and Utah dispensing License
DEA
BLS
Experienced with Ages 2+

COMPENSATION:
Competitive Market Rate (rate based on availability, experience, certifications, housing, and transportation needs, etc.)
Malpractice insurance provided

SCHEDULE:
ASAP - 4/25 Thurs each week - this could be on-going for Thursdays and possibly more each week. 7am to 5:30 pm. May need coverage on the other four weekdays in the future
