Physical Therapist - Home Health - (PT - HH)

Your caseload may include adults and primarily geriatric patients recovering from surgery or hospitalization, managing chronic conditions, or dealing with general deconditioning.A typical day will involve traveling within your assigned territory in and around Tucker to complete scheduled home visits. You can expect to manage a productive caseload that balances initial evaluations, routine follow-up visits, and reassessments. Documentation is completed using an electronic medical record system, and you will coordinate care with nursing, occupational therapy, speech therapy, and other team members to ensure cohesive, goal-driven care. Patient visit volumes and travel expectations are structured to support both quality care and manageable workloads.You will be responsible for monitoring patient progress, updating plans of care, communicating changes with the broader care team, and participating in case conferences as needed. The role may also involve occasional participation in performance improvement initiatives and clinical education within the team, allowing you to contribute to ongoing quality and practice standards.Schedules typically follow daytime hours with some flexibility based on patient needs and agency policies.
