Family Practice - Geriatrics Physician

Job description

Coverage Needed

  • Start Date: ASAP
  • End Date: Ongoing
  • Schedule:
    • Three days per week (preferably Tuesday, Wednesday, Thursday)
    • 7a-5p, 10-hour shifts
  • No Call

Clinical Details

  • EMR System: MediTech
  • Skilled Nursing Facility (SNF) and Long-Term Care (LTC) unit located on the campus of the medical center
  • Practice Settings: Outpatient, Inpatient
  • Patients Per Shift: 12-14
  • Rounding: assigned patients
  • Support Staff: 1 PA/NP (dedicated)
  • Beds: 99
  • Trauma Level: 3
  • Responsibilities:
    • Provide comprehensive medical oversight for both short-term rehabilitation patients and long-term care residents
    • Review and approve admissions to the Skilled Nursing Facility, including completing comprehensive history and physical examinations for all new admissions
    • Conduct required physician compliance and recertification visits
    • Provide ongoing primary medical care for long-term care residents, including the management of chronic medical conditions and preventative care
    • Develop, review, and update individualized care plans in collaboration with nursing, rehabilitation services, case management, and social work
    • Manage acute changes in condition, evaluate new symptoms, and provide treatment to reduce unnecessary emergency department transfers and hospital readmissions
    • Prescribe, monitor, and adjust medications
    • Participate in interdisciplinary care conferences, family meetings, and goals-of-care discussions
    • Complete required documentation, physician orders, certifications, and regulatory documentation
    • Coordinate transitions of care between the hospital, skilled nursing, long-term care, specialty services, and community providers
    • Provide ongoing medical oversight for long-term care residents who require continuous physician management
    • Routine medical evaluations and chronic disease management.
    • Annual wellness and preventive care assessments
    • Management of dementia, frailty, and other complex chronic conditions
    • Oversight of palliative and comfort-focused care plans
    • Collaboration with families and healthcare decision-makers regarding changes in condition, treatment plans, and advance care planning
    • Certification and documentation to maintain residents' long-term care plans and regulatory compliance

Requirements

  • Board certified or board eligible in Internal Medicine or Family Medicine
  • Licensed in Montana or IMLC
  • Certifications: ACLS, BLS
  • Credentialing Timeframe: 120 days
  • Experience caring for geriatric and medically complex patients (strongly preferred)
  • Experience in Skilled Nursing, Long-Term Care, Transitional Care, and Post-Acute Medicine
