• Brand-new clinic locations throughout the Phoenix metropolitan area

• Multiple openings available across eight clinics

• Provider will practice exclusively at one location:

  • Maryvale - Spanish bilingual required

  • Litchfield Park - Spanish bilingual required

  • Surprise - Spanish bilingual required

  • Deer Valley - Spanish bilingual required

  • Paradise Valley - English only

  • Tempe - Spanish bilingual required

  • Gilbert - Spanish bilingual required

  • Chandler - Spanish bilingual required

• Outpatient-only practice

• Care for patients ages 0-65

• Must be comfortable treating patients across the lifespan

• Commercial insurance patient population only

• Annual women's wellness exams required

• Monday-Friday

• 8:00 AM-5:00 PM

• No weekends

• No call

• No inpatient responsibilities (optional hospital privileges available for referral purposes)

• Gradual ramp-up to approximately 17-21 patients per day
