Schedule:
Start: May 18 or ASAP (post credentialing)
Month-to-month (option to extend)
Monday Friday, 8:00am 5:00pm (1-hour lunch)
Full-time only (40 hours/week)
Assignment Details:
70% telehealth / 30% home health (within 30-mile radius)
Adult + pediatric patients (must be comfortable with both)
Telehealth: patients/day
Home visits: 6 7/day (mixed avg 10/day)
Assessment-only role (no prescribing/treatment)
Annual risk assessments + condition reviews
Escalate urgent findings to PCP
Equipment provided by client
Schedules released 2 3 weeks in advance
Paid full day regardless of patient volume (must log availability)
Requirements:
Family Nurse Practitioner
Active MS license
Must be credentialed/enrolled in Medicaid
Comfortable working independently
