Hospice/Palliative Medicine Nurse Practitioner

Job description

Become part of a community hospice organization as a Nurse Practitioner, collaborating closely with the Medical Director, Hospice Physicians, and interdisciplinary team to deliver high quality, compassionate care to patients and families across diverse settings.

The Position:

  • Schedule is Monday Friday 8 am to 4:30 pm
  • Provide direct clinical care including assessments, diagnosis, treatment planning, and follow-up care.
  • Complete hospice face-to-face visits and eligibility evaluations.
  • Provide care in the IPU and in patient homes.
  • Conduct routine IPU rounding including symptom management and documentation.
  • Prescribe medications and order treatments within scope of practice.
  • Collaborate with physicians and interdisciplinary teams for coordinated care.
  • Educate patients, families, and staff on hospice care.
  • On-call coverage is required of at least one weekday per week from 4:30 PM to 7:00 AM and one weekend per month from 4:30 PM Friday to 7:00 AM Monday.
  • Territory includes Manitowoc, Sheboygan, & Ozaukee County.

Apply if you Have

  • An Active and Unencumbered Nurse Practitioner License in the state of Wisconsin
  • A master s degree in nursing
  • Strong clinical judgment, communication, and interpersonal skills with the ability to work effectively with diverse populations
  • Professionalism, tact, and sound decision-making in all interactions
  • Clinical expertise in medications, sterile techniques, and safety practices (PPE)
