Nurse Practitioner (Family / Primary Care) MinuteClinic Full-Time Collaborative Care Model

We are seeking a compassionate and driven Family Nurse Practitioner (FNP) to join our MinuteClinic team. This is an excellent opportunity to provide high-quality, evidence-based care in a patient-centered, collaborative environment with a strong focus on outcomes and preventive care.

Position Summary

As a MinuteClinic Nurse Practitioner, you will work closely with a dedicated clinical care team to deliver comprehensive care to a panel of patients. This includes accurate assessment, diagnosis, treatment, and long-term management of both acute and chronic conditions, while promoting overall wellness and patient engagement.

Key Responsibilities
  • Provide primary and family evidence-based care , including assessment, diagnosis, treatment, and health counseling
  • Manage acute and chronic health conditions with a focus on quality outcomes
  • Perform routine exams and diagnostic testing
  • Educate patients on healthy lifestyle choices and preventive care
  • Collaborate with healthcare professionals to ensure comprehensive care plans
  • Encourage patient participation through health screenings, behavioral health, and wellness programs

Administrative Duties:

  • Open and close clinic operations
  • Manage inventory and maintain a clean, organized environment
  • Handle patient appointments and follow-ups
  • Verify insurance and collect payments
Qualifications & Experience
  • Master s Degree from an accredited Family Nurse Practitioner program
  • Current National Board Certification and active state NP license
  • Minimum 1 year of recent Primary Care experience preferred (Family Medicine or Primary Care)
    • Newer providers may be considered with completion of MinuteClinic primary care training program
  • Strong understanding of chronic disease management and quality metrics
  • Self-motivated with strong problem-solving and organizational skills
  • Excellent communication skills (verbal, written, and electronic)
  • Ability to work independently and autonomously
  • Comfortable using technology and EMR systems
  • Willingness to obtain multi-state licensure
  • Proven ability to collaborate with interdisciplinary teams
Schedule M-F 8:30-7:30, Sat 9-5:30, Sun 9-4:30. Every other weekend. Training is M-F 40 hours x three weeks
