Family Practice/Primary Care Nurse Practitioner

Job description

We are seeking a dedicated and experienced Nurse Practitioner (NP) to join a collaborative clinical care team providing high-quality, evidence-based care in a primary care setting. This role offers flexibility, autonomy, and the opportunity to make a meaningful impact on patient outcomes.

Position Summary

As a MinuteClinic Nurse Practitioner, you will work alongside a supportive clinical care team to deliver comprehensive, patient-centered care. You will be responsible for assessment, diagnosis, treatment, and ongoing management of acute and chronic conditions while promoting preventive health and wellness.

Key Responsibilities
  • Provide evidence-based primary and family care , including assessment, diagnosis, treatment, and health counseling
  • Develop and manage comprehensive care plans for patients
  • Perform routine exams and diagnostic testing to support clinical decisions
  • Educate patients on healthy lifestyle choices and disease prevention
  • Support chronic condition management and preventive screenings
  • Coordinate care with other healthcare professionals
  • Encourage patient engagement in their own health and wellness

Administrative Duties Include:

  • Opening/closing clinic operations
  • Inventory management and maintaining clinical supplies
  • Patient follow-ups and appointment management
  • Insurance verification and payment collection
  • Ensuring a clean, safe, and organized clinical environment
Requirements
  • Master s Degree from an accredited Family Nurse Practitioner program
  • Current National Board Certification and active state NP license
  • Minimum 1 year of recent Primary Care experience (Family Medicine, Primary Care, or Urgent Care)
  • Strong understanding of chronic disease management and quality metrics
  • Ability to work independently and autonomously
  • Excellent communication, organization, and multitasking skills
  • Comfortable with technology and electronic medical systems
  • Willingness to obtain multi-state licensure
  • Ability to work in a remote collaborative care team environment
Schedule & Work Environment
  • hours per week
  • Rotating schedule including every 3rd weekend
  • Float position across locations
  • Individual start
