- Provide physical therapy services to patients in a home health setting within an assigned territory in North Napa County
- Meet a minimum productivity expectation of 30 points per week
- Complete all clinical documentation by 10:00 AM the following day
- Participate in a weekend rotation as part of the standard scheduleRequired Qualifications
- Active Physical Therapist (PT) license in the state of California
- Minimum of 1 year of home health experience
- Proficiency with HCHB (Homecare Homebase) software is requiredPreferred Experience / Skills
- Familiarity with home health territory management and patient scheduling
