Position Overview

We are seeking an experienced Primary Care Nurse Practitioner to provide on-site medical assessments for residents in a nursing home setting. This is a part-time opportunity ideal for a provider looking for consistent, predictable hours without call or weekend responsibilities.

Responsibilities
  • Conduct comprehensive and follow-up medical assessments for nursing home residents

  • Manage chronic conditions and address acute medical concerns as needed

  • Perform routine evaluations, medication reviews, and care plan updates

  • Collaborate closely with nursing staff and facility leadership

  • Document all patient encounters accurately and in a timely manner within the EMR

Schedule & Coverage
  • On-site coverage 2 days per week

  • No nights, weekends, or call

  • Flexible scheduling depending on provider availability

Qualifications
  • Certified Nurse Practitioner (NP) with Primary Care focus (FNP, AGNP, or equivalent)

  • Active state NP license (or ability to obtain)

  • Experience in primary care, geriatrics, or long-term care preferred

  • Strong assessment, documentation, and communication skills
