Requirements

• Active state license in hand
• Active LA DEA and CDS in hand
• Board Certified
• Residency Trained in EM/FM/IM
• ATLS, ACLS, PALS required if not ABEM-certified
• Flu and other immunizations required
• Credentialing Timeframe: 60-90 days
• Expedited Locum Privileges available with a clean file
