Nurse Practitioner - Geriatrics Fayetteville, North Carolina Key Candidate Requirements

Licensure: Must be a NP licensed in North Carolina
Experience: Minimum prior clinical experience required; fresh graduates not accepted
Certifications: Current BLS/ACLS required
Availability: Full-time, participation in on-call rotation
Visa Status: PERM only; no locums or visa candidates

Clinical Role

Provide comprehensive, patient-centered primary care to geriatric and frail populations
Collaborate with PACE Organization physicians to deliver primary care services
Perform comprehensive participant assessments (initial, ongoing, as-needed)
Develop, implement, and manage individualized care plans
Evaluate and treat acute illnesses
Participate in discharge planning for hospital or long-term care transitions
Conduct home visits as needed for continuity of care
Assist in developing and implementing clinical policies and procedures
Actively participate in interdisciplinary team (IDT) meetings
Ensure accurate, timely documentation of all clinical encounters
Support a compassionate, mission-driven care environment

Practice Setting

Patient Population: Frail and elderly patients in a PACE (Program of All-Inclusive Care for the Elderly) setting
Team: Collaborative interdisciplinary care team
