    Job Requirements
  • Must have current professional license with verification within 30 days prior to assignment start.
  • Completion of Ascension Core Competencies including HIPAA, Joint Commission, NPSG, OSHA topics with minimum 80% passing score.
  • Must complete Ascension Medical Imaging Contract Worker Onboarding Education Attestation.
  • Must provide Ascension Badge Photo (forward facing headshot with white or blue background).
  • Documentation of health requirements including MMR, Fit Test, Hepatitis B vaccination or declination, Health Statement, 2 Step PPD or IGRA, Varicella vaccination or immunity, Tdap valid within 10 years.
  • Must pass Ascension 12 Panel Drug Screen within 30 days prior to start and annually thereafter.
  • Must provide Ascension Profile (resume), two professional references, criminal background check results, EPLS check, education verification, confirmation of assignment, and employment verification.
  • Must complete Ascension Skills Checklist for specialty within one year of start and update annually.
    Schedule Information
  • Shift hours: 07:45 - 17:00 Monday through Thursday, 07:45 - 13:00 Friday.
    Additional Details
  • Ascension Resume required for submission.
  • Signed Assignment Agreement required for onboarding.
  • Influenza vaccine is optional; if declined, mask wearing required during flu season (Oct 1 - Mar 31).
  • Positive PPD requires negative chest X-ray and annual TB questionnaire.
  • Driver's License/State ID must be current and match address on file.
  • Hiring Manager Evaluation and Name Change Document are optional.
