ARRT or ARMRIT certification required
Primary Source Verification of professional license and/or certification within 30 days of start date
MA Conditional Licenses not accepted
Must have documentation of TB skin test screening within 3 months of start date and updated if >6 month break
Documentation of two measles vaccines, two mumps vaccines, and one rubella vaccine or proof of immunity via blood test
Fit Test (N95) within 365 days to start date and updated annually
Respirator Medical Clearance (RMC) Attestation required if fit test is required
Physician Health Statement signed by MD/DO/NP/PA within 365 days of start date and updated if >6 month break
MGB Criminal Background Check within 45 days prior to start date and updated if >6 month break
MGB Massachusetts CORI background check within 30 days prior to start date and updated if >6 month break
MGB Resume including past 7 years of work experience required for submission
Two references within 4 years of submission, minimum 1 supervisor or above in skillset
MGB Skills Checklist must be completed within 1 year of submission
MGB Confirmation of Assignment/POI Form required for all initial assignments and extensions
MGB Education Verification through background check company
MGB SSN Trace must be run with criminal check
MGB National Sex Offender Check ran within 30 days of start and updated annually
MGB Exclusion Checks ran within 30 days of start and annually

MA Conditional Licenses not accepted
No fit testing (initial or updates) can be done onsite at any location
Fit Test must be completed on the MGB N95 Respirator Fit Test Record Form
No religious or medical exemptions allowed for Fit Test
Updates for Fit Test can be done at Martha's Vineyard & Nantucket if traveler extends beyond original assignment
Strongly preferred Hepatitis B vaccination or proof of immunity or declination
Positive PPD requires negative chest x-ray documentation
MGB Name Change Documentation required if credential documentation does not match current legal name or profile
MGB TB Symptom Survey required if Positive PPD, to be completed prior to start and updated annually
MGB Candidate Submission Coversheet required for submission to initial assignment and new facility
