Aboutthe Role

We'relooking for an experienced, autonomous Nurse Practitioner to join our clinicalteam part-time, providing primary and episodic care to residents in skillednursing facilities. This is an independent role built around continuity -you'll manage your own panel and build long-term relationships with patientsand their families. New grads welcome!

YourDay-to-Day

You'llsee roughly 16-20 patients a day - a mix of long-term chronic care and acuteissues. That means annual wellness visits and H&Ps, managing chronic andco-morbid conditions, responding to acute changes, reviewing meds and orders,and coordinating with collaborating physicians, specialists, and facility staffto keep care seamless. You'll also stay in touch with families and join careconferences where it makes sense.

Qualifications

  • Board certification required (e.g., AGNP, FNP) - active and unrestricted.
  • Valid, unrestricted NP license in the state of operation.
  • Master's degree in Nursing from an accredited program.
  • Skilled nursing or long-term care experience strongly preferred.
  • Strong independent clinical judgment and the ability to move quickly in a fast-paced setting.
