Minimum Requirements:
- Must have State License to Apply
- 6 months Pharmacist experience preferred
- EPIC experience
- Medication reconciliation for discharge medication list
- Experience gathering prior-to-admission home medication lists from patient, family, or pharmacy
- Experience with discharge counseling for patients and families
- Clinical knowledge of antimicrobial stewardship, including interpretation of culture results for recommendations
- Clinical knowledge of UTI, pneumonia, and SSTI for recommendations
- Pharmacokinetic dosing management and monitoring
- Anticoagulation dosing management and monitoring
