Job description

Coverage Needed:

  • As soon as credentialed - Ongoing
  • Must be available 2- 3 Days of week to work in clinic
  • Clinic Schedule: Monday - Friday, 8am-4:30pm
  • No call

Clinical Information:

  • New Patients in follow up visits
  • EMR: EPIC (experienced preferred, but will train)
  • Knowledge of NCCN guidelines
  • Emergency response skills from side effects of medication
  • Chemotherapy treatment plans
  • Patient volume: 20 patients/day in clinic
  • 2 Physicians/ 2 APPs in practice
  • Support Staff: 3 RNs and 2 MAs

Required:

  • Board Certified
  • Must be Payor enrolled
  • Active IN License and DEA (or one that is easily transferable)
  • CV must be in Month / Year format
  • Must have license in hand at time of submission, or proof that the license application has been submitted
  • Any gaps greater than 30 days will require an explanation on the CV
  • DEA must remain in state to maintain privileges. Provider must be willing transfer and maintain DEA in state for the duration of privileges being active, or obtain a new in state DEA to remain in state for the duration of privileges being active
