  • Facilitates the efficient transfer of patients between facilities.
  • The Transfer Center Coordinator is a Registered Nurse (RN) responsible for coordinating the safe, timely, and efficient transfer of patients into, out, and within the healthcare system.
  • This role serves as a central point of communication between referring facilities, physicians, nursing leadership, bed management, and ancillary departments to ensure appropriate placement of patients based on clinical needs, bed availability, and organizational capabilities.
  • This role ensures compliance with regulations and maintains detailed records for quality assurance.
  • Previous Epic experience within the last 12 months required.

  • Night shift: 19:00 - 07:30.
    Additional Details
  • Influenza Vaccine: Season Sept. 1 - March 31st. Vaccine or facility declination accepted, due by 12/01 annually.

  • MMR: 2 vaccines or positive titer; if low or equivocal, 1 booster or declination accepted.

  • TB Test: Proof of negative test within past year; if history of positive test, additional documentation required.

  • Driver's License/State ID: Must be current and valid during assignment.
  • Varicella: 2 vaccines or positive titer; if low or equivocal, 1 booster or declination accepted.
  • Tdap: Valid for 10 years from administered date; declinations accepted.
  • GRH Background Check: Within 30 days of start; includes SS trace and county search for last 7 years.
  • GRH Iowa Adult and Child Abuse: State requirement.
  • GRH Education Verification: Verified through 3rd party; combine with diploma/transcripts if needed.
  • GRH EPLS/GSA/SAM HHS/OIG: Ran within 30 days of start.
  • GRH References: 2 professional references required, 1 must be from supervisor.
  • GRH Drug Screen: Within 30 days of start; 9 panel excluding THC; instant accepted if results unavailable.
  • GRH Skills Checklist: Complete within 12 months; does not expire.
  • GRH SING: Ran within 30 days of start.
  • GRH Resume: Required.
  • GRH I9 Attestation: Required on agency letterhead.
  • GRH Color Vision: Required.
  • GRH Contract Employee Data Sheet: Required.
