Nurse Practitioner / Physician Assistant - Primary Care

permanent, full-time Nurse Practitioner / Physician Assistant opportunity created due to organizational growth. The role focuses on providing high-quality, patient-centered primary care to
patients of all ages.3 office locations all in Salinas, California.
Monday-Friday - 7:30 AM - 7:00 PMSaturday - 8:00 AM - 2:00 PMSunday - Closed.
- Take call based on schedule arranged by the office
- Must be available to work some evenings and Saturdays
- Patient population: All ages
Primary Responsibilities- Perform comprehensive health assessments and scheduled follow-up visits
- Review labs, vitals, and symptoms; adjust treatment plans accordingly
- Provide preventive care, lifestyle counseling, and disease management education
- Support highly complex patients, helping them navigate the healthcare system
- Deliver one-on-one patient-centered care
- Coordinate care between patients, families, physicians, and care team members
- Administer appropriate tests and screenings
- Ensure accurate documentation, HCC-focused coding, and compliance
- Maintain strict HIPAA confidentiality standards
- Perform other duties as assigned
Candidate Qualifications- Active California Nurse Practitioner license
- Board certified NP (ANCC or AANP)
- Board Certified PA (NCCPA)
- 2+ years of NP experience in Primary Care, Family Medicine, Internal Medicine
- Strong knowledge of chronic care management and medication therapies
- Bilingual English/Spanish required
- Active CPR/BLS certification
- DEA license and prescriptive authority
Compensation & Benefits- Competitive compensation package; $100 - $110 per hour (based on experience)
