- $135,000 to $150,000+ Annual Compensation
- $60 to $65+ per Hour, negotiable based on experience

This compassionate non-profit healthcare organization is dedicated to delivering convenient, high-quality care to patients in a variety of settings, including long-term care facilities and private homes. They currently boast a team of 12 physicians, nurse practitioners, and physician assistants who, along with nursing, clinical, and administrative team members, ensure that patients receive the best medical care possible while working diligently to remove access barriers.
POSITION DESCRIPTION:
The Nurse Practitioner or Physician Assistant will be responsible for delivering comprehensive primary care on a regular basis to a dedicated panel of geriatric patients.
Patients are seen in private homes, assisted and independent living facilities, and other group settings.
The duties of the role include, but are not limited to:
- comprehensive physical examinations
- manage chronic conditions
- diagnose and treat illnesses
- advanced care planning discussions
- coordinate care with other providers
- palliative care consultations
Providers see an average of 6 patients per day. Based on patient need, more visits may be scheduled and are reimbursed per visit in addition to the hourly rate.
All patients are medically complex geriatric adults who may have:
- high clinical complexity
- multi-morbidity
- functional decline
- mobility limitations
- polypharmacy
- cognitive impairment
- increased risk for avoidable utilization
- evolving goals of care
- caregiver burden
This requires providers to build and maintain therapeutic relationships with patients and caregivers, including emotional support and education.
The practice uses the AthenaCare EHR.
REQUIREMENTS:
- North Carolina NP or PA license
- DEA
- 1+ year advanced practice experience required
- 2+ years advanced practice experience preferred
- previous home health or long-term care facility experience
- strong background in geriatric care
