We are looking for a compassionate and reliable LPN to join our growing Home Care agency. The ideal candidate has experience providing skilled nursing care in the home setting and is committed to delivering high quality, patient centered services. This role includes monitoring patient conditions, following care plans, completing accurate documentation, and communicating effectively with the RN Clinical Manager and care team.

Responsibilities include performing skilled nursing tasks, supporting patients with chronic conditions, ensuring safety in the home environment, and reporting any changes in patient status. The LPN will work closely with the clinical team to ensure continuity of care and a positive experience for every patient. We are seeking someone dependable, professional, and dedicated to helping our agency grow while providing exceptional care in the community.
