    Job Details
  • Performs patient assessments to determine appropriateness for hospice care and develops care plans for patients and families.
  • Ensures appropriate utilization of services and maintains visit productivity standards.
  • Completes discharge assessments and documentation within required timeframes.
  • Uses age-appropriate teaching methods for patients and families and documents education tools used.
  • Assumes responsibility for case management including developing, implementing, and evaluating plans of care.
  • Demonstrates knowledge of hospice regulations, insurance reimbursement guidelines, and pharmacology for safe medication management.
  • Participates in interdisciplinary team meetings every two weeks and communicates progress toward goals.
  • Maintains a safe environment by following infection control measures and reporting infections.
  • Participates in performance improvement programs and provides orientation and mentorship to new employees.
  • Assists with agency business growth and cross-trains to learn new skills.
    Job Requirements
  • Must have two (2) years of experience as an RN in a home hospice environment.
  • Experience using an EMR, excellent critical thinking and communication skills.
  • Valid New Jersey RN License required.
  • Valid New Jersey driver's license required.
  • BLS certification required.
  • Preferred: Three (3) or more years of RN experience.
  • Preferred: Previous ICU experience.
    Schedule Information
  • Shift: AM
  • Hours per day: 7
  • Hours per week: 37.5
  • Every other week (EOW) schedule
    Unit Specific Information
  • Home hospice environment
  • General In Patient (GIP) care
    Additional Details
  • Must have valid New Jersey driver's license.
  • Preferred ICU experience.
  • Participates in monthly staff meetings and performance improvement activities.
