We are seeking a compassionate Family Practice Physician for a 60-day assignment with our client in New York, commencing Aug 14, 2026. This role involves providing in-home primary care to a panel of adult and geriatric patients, focusing on comprehensive initial evaluations, routine follow-up visits, and the diagnosis, treatment, and management of chronic and acute conditions. The Physician will also manage medications, develop individualized care plans, and coordinate care with a multidisciplinary team, typically working 8:30 am to 5:00 pm shifts. Responsibilities and Duties Provide in-home primary care to a panel of adult and geriatric patients Conduct comprehensive initial evaluations and routine follow-up visits Diagnose, treat, and manage chronic and acute conditions Perform medication management, reconciliation, and adjustments Develop and implement individualized care plans Coordinate care with NPs/PAs, nurses, social workers, and care coordinators Order and interpret labs, imaging, and diagnostic tests Deliver preventative care, including screenings and wellness visits Manage transitions of care, including hospital and SNF discharges Communicate with patients, families, and caregivers regarding care plans Address urgent patient needs and triage issues during assigned coverage periods Additional Information Board certification in Family Medicine, Emergency Medicine, or Internal Medicine is required EMR: Athenahealth Patient visit lengths range from 30 to 60 minutes Management of chronic diseases including diabetes, hypertension, congestive heart failure, and COPD Geriatric care addressing frailty, fall risk, cognitive decline, and dementia Transitional care, including post-hospital and post-SNF discharge follow-ups Medication management and polypharmacy optimization Addressing acute low-acuity issues such as infections, minor injuries, and symptom flare-ups Preventative care, annual wellness visits, screenings, and vaccinations Behavioral health support for depression, anxiety, and medication adherence Wound care management for pressure ulcers and diabetic wounds Complex care coordination involving multiple specialists and services Benefits Strong compensation Travel-related expenses covered A-rated medical malpractice insurance provided Dedicated recruiter for future travel opportunities What are Locum Tenens Jobs?
